What "immediate" and "same-day" mean
These terms cover a few related ideas that are often blurred in marketing:
- Immediate placement The implant is placed at the same appointment the failing tooth is extracted, rather than waiting for the socket to heal.
- Immediate loading (same-day teeth) A temporary crown or bridge is attached to the implant straight away, so you leave with a tooth — though this temporary is not for heavy chewing while the implant heals.
- Same-day / "teeth in a day" Usually refers to full-arch cases like All-on-4 where a fixed temporary bridge is fitted the day of surgery.
The final, definitive crown or bridge almost always still waits for the implant to fully integrate over the following months. "Same-day teeth" means same-day temporary teeth in most cases.
Who it suits — and who it may not
Immediate protocols work best when you: have enough good-quality bone to achieve strong initial stability (primary stability) at placement, have healthy gums with no active infection at the site, and do not have heavy uncontrolled clenching. They may not be advisable if you: have significant infection around the failing tooth, poor bone quality or quantity, heavy smoking, or a bite that would overload a fresh implant. The dentist judges suitability from the CBCT and, crucially, from how stable the implant feels at the moment of placement.
Step by step (immediate case)
- 1. Planning and CBCT Confirming bone and site conditions in advance.
- 2. Extraction (if needed) The failing tooth is removed atraumatically to preserve bone.
- 3. Immediate placement The implant is placed into the socket or prepared bone; stability is assessed.
- 4. Temporary tooth (if loaded) If stability is sufficient, a temporary crown/bridge is fitted, kept out of heavy bite.
- 5. Healing Osseointegration over the usual months, on a soft diet where a temporary is worn.
- 6. Final restoration The permanent crown or bridge is fitted after integration.
A note on mini implants
Mini dental implants are narrow (usually under 3 mm) one-piece implants sometimes used to stabilise a lower denture or in very narrow spaces, and they are often placed and loaded quickly with less surgery. They can be a lower-cost, less-invasive option in specific situations — particularly denture stabilisation — but they are not a universal substitute for standard-diameter implants, may carry more load per implant, and have narrower indications. They should be presented as a specific solution for a specific problem, not a cheap shortcut for everything.
Materials, risks and timeline
The implants are titanium; the same reputable systems apply. The appeal of immediate protocols is fewer appointments and no long gap without a tooth. The trade-off is a slightly higher risk of implant failure than delayed placement if primary stability or site conditions are not ideal, and the discipline of protecting the temporary during healing. Risks otherwise mirror standard implants: infection, non-integration, and later peri-implantitis. For travellers, immediate protocols can reduce the number of trips, which is part of their appeal in medical tourism — but only when the case genuinely suits them.
